How successful are the mail out culture kits? Are they swabs? expectorants? Who teaches for a correct swab?
Questions for Ryan: Do you know what your response rate was (i.e., how many patients were asked to complete it vs. how many did), and 2. How did you solicit responses? We have had a low response rate and none of our request methods have worked tremendously well..
What do these home sputum kits contain? Anyone using these currently?

I have been told sputum must be refrigerated to prevent overgrowth. Are these overnighted? Who covers the cost of collection and delivery?
We do it and use FedEx overnight

I like the idea of setting the agenda verbally at the beginning of the visit (first provider). Great idea so the patient/family knows what to expect.

Same, Julianna!I believe the CFF said 1 out of 4 CF patients experienced food insecurity this year? (Don’t quote me, this is off the top of my head)
For a wrap up process, we tried this as a team based on the COPD/Asthma Action Plan document. We have changed our focus now to still use that Action Plan document but is for the patient to complete as they see fit. In this was we are involves them in the ongoing planning process. We like that - we have our clinic note for team actions and the Action Plan is the patient's note to follow-up on.

Question for Arrica: if you were to design the vulnerable population what would you change?
Hi, our clinic struggles with a population that is often non-compliant, how do you manage non-compliance with telemed patients? i.e not getting lab results, child is not on the video, did not follow through with previous instructions
