For a wrap up process, we tried this as a team based on the COPD/Asthma Action Plan document. We have changed our focus now to still use that Action Plan document but is for the patient to complete as they see fit. In this was we are involves them in the ongoing planning process. We like that - we have our clinic note for team actions and the Action Plan is the patient's note to follow-up on.
